When ordering the AMSDiet12PC program you must do, and include the following:

1. The name of the ordering individual
2. The individual's mailing address
3. The individual's email address
4. The recipient's full mailing, and/or E-mail address(if different).
5. The name and birthdate of the person for whom this product is ordered:
   1. The names and birthdates of up to three individuals allowed to use this program. No additional charge for these three additional individuals.
6. After your payment of $40.00 USD and the above required informations is received by Astro Metphysical services, your copy of this program will be E-mailed to the E-mail address you prefer!
